To The Editor:We read with interest the paper, "Effect of Bisphosphonates on Periprosthetic Bone Mineral Density After Total Joint Arthroplasty. A Meta-Analysis" (2005;87:293-301), by Bhandari et al. We were intrigued by the findings that the effect size of bisphosphonate therapy was greater after procedures performed with cemented implants than after those performed with uncemented implants and greater after total knee arthroplasty than after total hip arthroplasty. We noted that both of the total knee arthroplasty studies involved cemented implants but that half of the total hip arthroplasty studies involved uncemented implants. Would the authors clarify whether cementation and joint site were independent predictors of drug efficacy, or whether the greater effect after procedures involving cemented implants might have been due to the common variable effect of total knee arthroplasty?
